Welcome to the FVBSA Online League Sponsorship Form. Thank you for sponsoring our program. Your valued contribution will help the league with the costs of needed items such as player scholarships, equipment and field use fees.

For your convenience, we have provided two methods of payment:

Debit/CC Payment- If you will be paying via Visa/MasterCard debit/credit card, please answer the questions below on this form and submit your payment.

Check Payment- If you will be paying via check or cash, complete this form and select $0 and send to the address provided with your payment.

SPONSORSHIP LEVELS

French Valley has several sponsorship levels to fit your business needs. Please look below for a description of each level:

PLATINUM LEVEL-$800.00 Donation
- Individual custom-made business banner displayed each Saturday during the season.
- Business name, phone, web link and logo (if provided) listed on league home page of website during the season.
- Sponsor appreciation plaque.
- Have your own table to promote your business on Opening and/or Closing Day.
GOLD LEVEL-$600.00 Donation
- Business name and Logo printed on the league banner displayed each Saturday during the season.
- Business name, phone, web link and logo (if provided) listed on league home page of website during the season.
- Sponsor appreciation plaque.
- Have your own table to promote your business on Opening Day.
SILVER LEVEL-$400 Donation
- Business name and logo printed on the league banner displayed each Saturday during the season.
- Business name, phone, web link and logo (if provided) listed on sponsor page of website during the season.
- Sponsor Appreciation plaque
COPPER LEVEL-$200 Donation
- Business name & phone placed on sponsor page of league website.
OTHER DONATION AMOUNT-Any amount is accepted and appreciated! Thank you for your contribution.
