Dear friends,
Qualifying organizations in Athens-Clarke, Barrow, Jackson, Madison, Oconee, and Oglethorpe Counties may apply to be a Partner Agency with the Athens Area Diaper Bank. To qualify, the organization must be one of the following:
- a 501(c)(3) organization,
- religious organizations who agree to pertinent sections of our Partner Agreement,
- government organizations.
If your agency qualifies for partnership and would like to apply to be added to our waitlist:
- Go to our Partner Page and review the information listed and linked there. Pay particular attention to the Partner Handbook and (sample) Partner Agreement.
- After review, return to this application and complete all required questions.
- You are encouraged to create a Jotform account so that you may save your progress.
- Once your application is submitted, you can view the submission, but you will not be able to change your answers.
- Submissions will be added to the Waitlist in order of completed application. Leaving sections blank or unclear descriptions will not work in your favor.
- When funding becomes available, AADB will review applications and send invitations via email to the person completing this form, the Authorized Signatory, and the Liaison. The email will include links for the Authorized Signatory to digitally complete the Partner Agreement.
- If your organization will provide funding to support your diaper program, explain this in the application.
This application should take 15-30 minutes to complete, and you will need a few things:
- Your organization's mailing address, phone number, and email address.
- A digital copy of your organization's nonprofit status documentation.
- A digital version of your organization’s logo.
- An estimate of the average number of children you plan to serve each month with AADB diapers.
- A detailed description of your organization’s existing (or planned) diaper distribution program. Use this section to clarify if you will provide funding for your organization's diapers.
- Contact information for:
- Authorized Signatory- whomever is authorized to make formal agreements on behalf of the organization referenced in your IRS nonprofit documentation.
- Diaper Program Liaison- whomever will be the primary contact for managing diaper requests; supervising appropriate diaper distribution procedures (correct number per child and size, no breaking up packs, etc.); scheduling and reporting Partnership Requirements such as surveys, diaper drives, volunteer sessions, client surveys, etc.
