Note that all vendor applications will come from a City Jotform account for from the Special Events team directly. If you question whether an application that is circulating is from the City, please email Specialevents@moval.org to confirm. The City only requests vendor payments from ActiveNet. The City does NOT request payment through PayPal, Cash App, Google Pay, etc.
This form is not an application but a request to be on a list of vendors that are interested in being a part of future events. The Special Events Team will reach out to those that would suit the needs or themes of events.

NO CAMPAIGNING ALLOWED AT ANY CITY-HOSTED EVENTS.
