Please fill this form out prior to making your purchase in Infinite Campus.
A field trip is scheduled for our Kindergartners to vist Fernbank Science Center located at 156 Heaton Park Dr, Atlanta, GA 30307, USA. Please keep the follwoing in mind:
- Trip Date: Friday, April 17th
- Trip Cost: $20.00 (per scholar)
- The group will depart at 9:00 AM and is estimated to return at 1:00 PM.
*DIA will not accept verbal approval*

**This form should be completed by the parent/guardian. Payment can be made in infinite campus by anyone**
