Please complete this form if:
1. You booked in for a hearing test, or
2. You are anticipating trialling hearing aids
Our Free Hearing Test: Our free hearing test is offered to determine your viability for hearing aids based on any hearing difficulties you may be experiencing. We understand that if you are visiting us for the first time, you may simply be trying to find out if hearing aids are the right solution for you. Hearing tests are strictly free to determine the viability of hearing aids to overcome your problems.
Defining an Aidable Hearing Loss: For a test to remain free of charge, it must identify an "aidable" hearing loss. We define this as having at least three separate readings above 30 dB HL, alongside noticeable difficulties understanding speech in background noise.
Diagnostic Consultation & Routine Testing Fee (£75): Routine tests to measure further loss are chargeable if there is no intent to purchase at the time. A £75 diagnostic consultation fee applies under the following circumstances:
- Routine Monitoring: You are attending to measure your hearing levels but have no intent to consider buying hearing aids to resolve your difficulties.
- No Aidable Loss Identified: Your hearing test reveals a stable loss or normal hearing that does not meet our criteria for an aidable loss.
- Medical Referrals: We identify a medically referable condition. In this instance, the £75 fee includes a written summary report of our findings to share with your doctor or specialist.
Copies of Test Results & Audiograms (£150) Our free hearing test consultations include a detailed summary of your hearing loss, but do not include a physical or digital copy of your audiogram for business competition reasons. If you require a copy of your test results, a £150 release fee applies.

Free hearing aid trial: A free hearing aid trial may be offered. There is no obligation to purchase hearing aids following a trial. If you take part, you agree to return trial hearing aids safely (in person or by post at your expense). You are responsible for loss or non-return. Replacement cost is £150 per hearing aid.
Audio recording: Consultations may be audio-recorded to help us create accurate clinical notes. Recordings may be processed by a secure third-party transcription/AI service. We do not use recordings for marketing. Our copy is deleted once notes are finalised, although the third-party service may retain limited data under its own policies. You can choose not to be recorded and may withdraw consent at any time after submitting this form.
Eligibility: You must be 18 years or over.
By signing, you acknowledge and agree to all terms above.
