The Project Lifesaver Program is meant for individuals that are in need of monitoring and suffer from a medical need that requires mon
itoring, i.e. Alzheimer’s, Autism, Dementia, and so forth. The client must be under the car of a physician that has documented their medical need for the program.
The client must be unable to care for him or herself and must reside with the primary caregiver.
Project Lifesaver Transmitters will be placed with clients ONLY, at the request of a legally responsible party, i.e.:
1. Spouse
2. Son or Daughter, in the absence of a spouse
3. Parent
4. Family member having legal, primary caregiver responsibility over the client
5. Assisted living or nursing home administrator requiring transmitter for client to reside at the facility.
In the event that there is no spouse and there is a disagreement on placement, NO placement will be completed until requested by the family member or entity having legal authority via power of attorney or court order naming him/her as the responsible caregiver.
On or about the first Thursday of every month, a York-Poquoson Sheriff’s Office Representative will meet with the client to preform maintenance on the transmitter and ensure that proper functioning of the transmitter.
