Step 1: Gather Your Documents
Before you begin filling out the new employee form, please ensure you have the following documents ready:
- Scanned Copy of Your ID: This can be a driver's license, passport, or any government-issued identification.
- Void Check: For setting up direct deposit payments.
- Vulnerability Check: Obtain this document from your local police department.
- Immunization Records: Ensure your records are up to date.
- First Aid Certificate: A valid certificate of completion.
- TB Test Results: A recent tuberculosis test result.
- Professional Certification (e.g. AMI, MACTE or RECE) (if applicable)
Step 2: Scan Your Documents.
Ensure that all physical documents are scanned and saved in a clear and readable format. Supported formats typically include PDF, JPG, or PNG.
