SCHEDULING
To certify a patient for the Arkansas Medical Marijuana Registry, we must establish a bona fide physician-patient relationship with you. Therefore, at least 2 15-minute visits are required.
FEES
Visit 1, Telehealth visit to establish care: Insurance accepted. If opting for self-pay or your insurance is out of network with our group a fee of $99.99 is required.
Visit 2, Certification visit (must be done in person): $75.
In case of financial difficulty, please reach out to our team for a discounted rate. Price matching with other Arkansas Medical Card providers is also available.
After you receive your certification from our office, you will be required to pay $50 to the Arkansas Department of Health, through the website when you apply, or by check or money order if you are mailing your application.
PROCESS
When you book your first visit, MVP will request pertinent records if applicable to expedite the certification process.
Your second visit must be completed in person.
Our Arkansas office for in person consultations is located at 1116 S. Walton Blvd., Suite 152, Bentonville, AR 72712.
NEXT STEPS: You must apply to the state registry via mail or through ADH’s online application system.
Online application is preferred; users will upload a copy of the signed form and Arkansas driver's license, then submit payment to the state. Bring a smartphone or tablet to the visit if you need assistance.
A paper application is also available to print out here; It can be mailed in, along with the original Physician Written Certification, copy of DL, and check or money order for $50.
The ADH may call the physician’s office to verify the doctor’s signature.
IMPORTANT: The physician’s certification is valid for 30 days from the signature date. If you get a certification form and fail to submit it to ADH within 30 days, you must schedule and pay for another visit to get a new certification form.
The state reviews your application within 14 working days, and may contact us to verify it. When approved, they will notify you via the online portal or through the mail. Your card will be good for up to one year as determined by your doctor.
