Theatre Bug - Enrolment Form 2026

I give permission for Theatre Bug Mackay to use video footage and /or photographs of my child. This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs, and/or video of my child for use in materials that include, but may not be limited to; Printed material (e.g. brochures, programs and newsletters), Online and offline advertising and promotion; Videos and digital images such for use on Social Media. By signing this form, I acknowledge that I am giving unrestricted permission for my child’s image to be used in print, video and digital media. I agree that these images may be used by Theatre Bug Mackay for a variety of purposes and that these images may be used without further notification. I do understand that any identifying information including surname and location will not be used in conjunction with any video or digital images.

I consent to my child participating in Theatre Bug Mackay classes and activities. I understand that every endeavour will be made to contact me prior to any medical attention being given. If it is not possible to contact me, I hereby authorize the Theatre Bug Mackay team to seek medical attention (including treatment, emergency transport, hospitalization, anaesthesia and medication) in the event of an accident mishap of illness during my child’s participation in the classes throughout 2026.

I understand that there is an annual enrolment fee that covers public liability and music licensing for 2026 as well as the regular term fees. I understand that an enrolment fee and term fees are non-refundable and non-transferable. I understand that if my child misses a group lesson, no make up lesson will be provided or a refund. I understand that if less than 24 hours notice is given for a missed set weekly private lesson the payment still needs to be made and not refunded. Payment will be accepted via electronic funds transfer or cash.
