This form is required for an application to be considered complete. If the applicant is a United Methodist Church, the applying church should complete this form. If the applicant is a non-profit organization, the pastor of the UMC involved with the program should complete the form. Please contact Stacey Rushing at director@nghhc.org with any questions.
