Instructions:

- Enter your valid UMID and Uniqname in the form below (the fields are at the bottom under "Important Information"

- Your order is limited to 1 item.

- Ordering is open until January 31st. However, we recommend that you place your order as soon as you receive this email, as we will be fulfilling orders in the order they were received. Due to processing time, please hold status inquiries until after January 15th.

Other Important Information:

- Place your order ASAP! Orders will be fulfilled in the order they were received.

- No payment is necessary. Please make sure to provide a valid shipping address.

- All items will be shipped to the addresses provided by you during checkout. You will receive tracking information to your UMICH email address upon shipping. No apparel will be available for pickup at local Underground Printing stores.

- Please note, once your order has been placed, you will receive a confirmation email which will include an order number. If you have not received a confirmation email within 24 hours, please contact michiganmedicinerecognitionorders@undergroundshirts.com ASAP, no later than January 31st, 2024.
