Volunteer Registration & Waiver Form

A Parent or Guardian (adult family member) must complete this registration process.

You only need to complete this registration process for your household once.

You will receive an email after this registration with instructions to add members to your household. All adults and children who will be participating with Generation SERVE must be registered. Adults are anyone (spouse, grandparents, aunts, uncles, etc.) over the age of 18 who might attend a volunteer activity with a minor. 

Demographic Information

This section asks for demographic information. This data helps us better understand who we are serving and also helps us secure future grant funding, and expand our program reach throughout the community. All demographic questions are optional, but we appreciate your responses. Your answers to these questions are strictly confidential and will only be shared when aggregated with other information.

Household Information

This section asks for information that will help Generation SERVE better promote our programming, as well as identify potential giving match opportunities.

 

Permission and Waiver of Liability Agreement

**Important: parents/guardians must complete this Permission and Waiver of Liability Agreement as part of the registration for participation in our volunteer activities. This agreement applies to all members of your Household (all family members - adult and minor children registered within your household account).

Generation SERVE wants registrants in our programs to be aware in advance that, when they participate in certain volunteer activities, there is a natural element of risk of injury that each participant must assume. In order to provide you and your family with meaningful volunteer activities, we require program participants to execute this Permission and Waiver of Liability Agreement. Generation SERVE does not carry medical or accident insurance for program participants because the cost of that type of insurance would make program fees associated with our efforts to coordinate volunteer activities prohibitively expensive. Please review your own personal health insurance plan to be certain that you and your family have proper coverage.

This Permission and Waiver of Liability Agreement is part of the registration for participation in all sponsored activities. You may also occasionally be presented with additional release forms from our partner/affiliate organizations that may be required based on the particular risks involved in the volunteer activity for which you register to participate. If so, that additional material or agreement will become part of this Permission and Waiver of Liability Agreement.

Volunteer Participation
By executing this agreement I am granting my child(ren) permission to be a minor volunteer for participation in activities sponsored or coordinated by Generation SERVE. I understand that all participation by me or members of my Household in activities coordinated or sponsored by Generation SERVE is voluntary and will be performed without any expectation of compensation, insurance coverage or cost reimbursement.

Assumption of Risk
As a volunteer, parent and/or guardian, I recognize that there are certain risks associated with participation by me and members of my Household in volunteer activities, and I agree to assume the full risk of any bodily injury, personal injury, illness, death, or property damage that may result from participating, in any manner, in activities coordinated or sponsored by Generation SERVE.

Release and Waiver of Liability
I release, waive and hold harmless Generation SERVE, its partners, affiliates, directors, officers, employees, agents, and independent contractors, from any liability or claim resulting from any bodily injury, personal injury, illness, death, or property damage that I or members of my Household may incur as a result of participating in volunteer activities sponsored or coordinated by Generation SERVE and its partners or affiliates. I also agree to indemnify, hold harmless and defend Generation SERVE and all released parties against any and all claims, demands, losses, causes of action, damage, lawsuits, judgments, including attorneys' fees and costs, that may arise in connection with my conduct or the conduct of members of my Household in connection with our volunteer service.

Healthy and Safety Waiver

Generation SERVE asks that you do not attend an activity if you are ill.

We request that any Generation SERVE participant report to us if they develop symptoms or test positive for COVID-19 within 14 days of attending a Generation SERVE event. We will attempt to alert other participants of possible exposure while maintating confidentiality.

Generation SERVE cannot guarantee that the participants in its volunteer activites will not be exposed to or become ill from infectious diseases, including but not limited to MRSA, influenza and COVID-19 (collectively, "Diseases"). There is an inherent risk of exposure to Diseases that exists in any public place where people are present. By signing this agreement, I acknowledge the contagious nature of Diseases and voluntarily assume the risk that I and any minor whom I accompany as parent or guardian may be exposed to or infected by participating in Generation SERVE volunteer activities and that such exposure or infection may result in personal injury, illness, permanent disability, or death. I voluntarily agree to assume, for all members of my Household, all of the foregoing risks, and accept sole responsibility for any injury to members of my Household (including, but not limited to, personal injury, illness, disability, and death), and assume damage, loss, claim, liability, or expense, of any kind, that members of my Household experience or incur in connection with my participation in Generation SERVE programming.

 

Photographic and Video Release and Intellectual Property Rights

I grant full permission and rights to use, without compensation, photographic images and videos of members of my Household and quotations made by members of my Household relating to our volunteer service and event participation in materials, advertisements, or other promotions for Generation SERVE. I understand that it is the policy of Generation SERVE to use only first-names, pseudonyms, or de-identified images, videos, or quotations in its materials to help protect the privacy of members of my Household.

Signature

Plese read this disclaimer/policy carefully, as it affects your legal rights. When finished, check the Waiver Agreement box and sign below. I have read and understand the above Permission and Waiver of Liability Agreement. I understand that Generation SERVE does not and cannot provide insurance or protection against injuries sustained by program participants, and I fully accept the risk of injury that may be associated with participation in volunteer activities. I also understand and agree that this document is valid in and of itself as a waiver and release of liability and discharges any and all claims of liability that I or members of my Household may result from our volunteer service. This Agreement is governed by Texas law, and any provision that may be held invalid by a court shall not affect the enforceability of the remainder of the Release.
