If you make the decision you would like to ‘adopt’ the dog as your own, PLEASE CONTACT US IMMEDIATELY TO SIGN AN ADOPTION CONTRACT – we understand that this happens and are glad to work it out with you and the dog.

We appreciate your interest to foster one of our dogs. Our ultimate goal is to find a perfect match for our dog and you. A PERFECT MATCH is placing our dogs in a foster home where the foster will provide a healthy, safe, secure and loving environment and where the dog is able to live until adopted out. Fostering requires caring for the foster dog physically and addressing medical needs (medicine and administration training will be provided, if necessary). The completed Foster Application is a contract, therefore all provided information in this application must be answered correctly. Completing the application is not an approval to become a foster. When you get the approval to foster the dog, you are taking over the responsibility and liability for all incidents that might occur while the dog is in your care and the rescue cannot be held responsible for.

I understand that Woof Den Rescue cannot guarantee the health or disposition of any foster animal. I understand that the rescue may not have complete past records for these animals and there are some risks associated with taking in foster animals. I understand the importance of my own animals being fully vaccinated. Family pets will be current on all shots and foster pets will be kept isolated from family pets for a minimum of 7 days for the protection of all animals. I agree to be fully responsible for the safety and health of my foster animal(s). I will provide a safe, loving, and humane environment at all times. I will adhere to all state and local animal laws. I will promptly notify my designated Woof Den Rescue contact of any signs of illness, behavior issues or concerns, an inability to foster, and if the pets become lost or if the pet bites someone. I agree to follow all policies and procedures.

I understand that a foster is not allowed to make commitments to give the dog to another party, list the dog in any public forum or paper, turn it into a shelter or adopt it out on their own. The rescue has to be notified immediately if something happens to the dog, if it becomes, sick, lost, stolen or injured. Without the rescue’s approval, no expenses spent for the dog are refundable. A foster has the responsibility and liability for the dog while in foster care, this includes public places, the foster home and everywhere the foster takes the dog as well as keeping the dog and any other party safe and secure by having the dog always on appropriate collar, harness, leash etc. and keeping yard/property secure with appropriate fencing and gates so the dog cannot escape. I agree not to leave a dog unattended or unleashed outside or out of eyesight and to avoid loss situations arising while the dog is in foster care.

I understand that all animals in Woof Den Rescue foster program are the property of Woof Den Rescue and must be returned within 24 hours of request. I agree that I am fostering this animal for Woof Den Rescue and do not have any right to ownership over my foster animal. I agree to provide an authorized representative of Woof Den Rescue access to my home and property to check on the well- being of my foster animal at any time while I am in possession of my foster pet. I also agree to random home visits to assure that I am maintaining a safe and reliable home environment for the animals that Woof Den Rescue is entrusting me to care for.

I agree that accidental animal bites or other injuries to humans and other animals do occur and agree to hold harmless and indemnify and protect Woof Den Rescue from any claim or suit filed by someone as a result of such an incident.

In addition, Woof Den Rescue will not be responsible if an animal should damage or destroy property belonging to Foster Parent or if the animal should transfer any disease, internal or external parasites to other animals and/or people in the Foster Parent's household.

I understand that if I am approved for fostering, this declaration represents a legal contract between me as the foster caregiver and Woof Den Rescue. I understand that if I am approved to foster an animal, I must abide by this agreement and that this agreement applies to any and all animals I foster for Woof Den Rescue .

By signing below, I acknowledge that the information I provided on this application is correct to the best of my knowledge. If at any time the information I have provided changes, I will provide the updated information to Woof Den Rescue .
