Event: The Barn
Date: June 14th 2024

In consideration of being allowed to participate in the all activities at The Barn organized by He Will Restore Ministries, I hereby acknowledge and agree to the following:

Assumption of Risk: I understand that some activities involve physical activity and may pose certain risks, including but not limited to minor injuries, accidents, and unforeseen events.

Voluntary Participation: I voluntarily choose to allow my student(s) participate in any/all activities and understand that my participation is not required.

Release of Liability: I hereby release, waive, and discharge He Will Restore Ministries, its officers, employees, volunteers, and any associated entities from any and all claims, demands, actions, or causes of action arising out of my student(s) participation in any games or activities offered at the venue.

Responsibility for Personal Property: I acknowledge that each student is responsible for my personal belongings during the event and release He Will Restore Ministries from any liability for loss, damage, or theft of personal property.

Medical Treatment Authorization: In the event of an injury or medical emergency, I authorize He Will Restore Ministries to seek and consent to medical treatment on the behalf of the student(s) registered.

Photographic Release: I grant He Will Restore Ministries the right to use photographs and/or videos taken during the event for promotional and marketing purposes.

Compliance with Rules: The student agrees to comply with all rules and instructions provided by He Will Restore Ministries and its representatives.
