    WHO WE ARE?

    Founded in 1968, Cornerstone Community Development Corporation is a minority, community based not-for-profit organization, located in the Village of Ford Heights, about 30 miles south of Chicago, Illinois.  Our core mission is to improve the quality of life for the underprivileged and to assist vulnerable populations in times of crisis, emphasizing emergency, supportive and educational services to individuals and families until they stabilize. 


    Cornerstone CDC's client base is primarily comprised of low income African Americans across South Suburban Cook County and East Will County, as well as parts of Chicago’s south side.   Cornerstone CDC implores an integrated, one-stop center model to abate the issues of homelessness, hunger, unemployment, and poverty that plague our surrounding communities.  Many of communities in our service are food deserts and rely on our multitude of anti-hunger programs..

    Overview:

    Cornerstone Community Development Corporation is seeking a passionate and dedicated Social Worker to join our team. The Social Worker will be responsible for providing support, advocacy, and intervention services to individuals and families in need within our community. This position offers the opportunity to make a positive impact on the lives of those facing various social and economic challenges.


    Responsibilities:

    1. Conduct comprehensive assessments of clients’ needs, including their social, emotional, and economic circumstances.

    2. Develop and implement individualized care plans based on assessment findings, in collaboration with clients and their families.

    3. Provide counseling, guidance, and support to individuals and families facing challenges such as poverty, homelessness, domestic violence, substance abuse, and mental health issues.

    4. Connect clients with appropriate resources, including housing assistance, healthcare services, employment opportunities, and community support programs.

    5. Advocate on behalf of clients to ensure their access to essential services and benefits.

    6. Collaborate with other social service agencies, healthcare providers, and community organizations to coordinate care and support for clients.

    7. Maintain accurate and confidential client records, progress notes, and statistical data in compliance with organizational and regulatory requirements.

    8. Participate in staff meetings, training sessions, and professional development activities to enhance knowledge and skills in social work practice.

    9. Contribute to the development and implementation of outreach programs and community initiatives to address social issues and promote positive change.

    10. Provide crisis intervention and support during emergency situations or times of heightened need.


    Qualifications:

    1. Bachelor’s or Master’s degree in Social Work or related field from an accredited institution.

    2. State licensure or certification as a social worker, if required by the state.

    3. Proven experience in providing direct social work services to diverse populations, preferably in a non-profit or community-based setting.

    4. Strong understanding of social justice issues, trauma-informed care, and culturally competent practices.

    5. Excellent communication, interpersonal, and organizational skills.

    6. Ability to work independently and as part of a multi-disciplinary team.

    7. Proficiency in using technology for documentation and communication purposes.

    8. Commitment to upholding ethical standards and maintaining professional boundaries in client interactions.

    9. Flexibility to adapt to the changing needs and priorities of the organization and the community.

    10. Passion for social justice, empathy, and a genuine desire to empower individuals and families to overcome life's challenges.
