  • By clicking submit, I hereby seek to participate in a healing circle with Rita Bozi and her assistant(s). I understand that they do desire to ensure the safety of all participants.  In order to assist them in this regard, I agree to complete the Confidential Medical History form provided.  I am assured that the information provided will remain strictly confidential and will serve only as a guide in determining the appropriateness of my participation in the healing circle and in meeting my needs before, during and afterwards. I have been informed of the nature of the event, the needed preparation and the rules of engagement. I commit myself to stay in the circle until the end of the workshop and to respect the directives given by the organizers, helpers and leader(s).

  • Participation in the healing circle can involve dramatic experiences accompanied by strong emotional and physical release. This healing circle is not appropriate for persons with certain medical conditions or for persons using medications. If you have any doubt or concerns about whether you should participate, consult with Rita before attending.
     
    Please answer the following questions as completely and honestly as possible. Your responses will remain strictly confidential.

  • Please be aware that the list of contraindicated substances includes, but is not limited to, SSRIs, Demorol, cold medication, decongestants, sinus medication, nasal sprays, hay fever medications, diet pills, amphetamines, MDMA or ecstasy, cocaine, heroine, and crack. If you have been using any drugs, medical, over the counter, or street, please advise us prior to attending any ceremony to discuss this matter further.
     
    Please note that ceremonies should not be seen nor are they designed as a substitute for psychiatric or other medical care.

  • Should be Empty:
