Doctor Consultation with a Developmental Pediatrician (or Dev Ped)

• Only Dev Peds can give your child a diagnosis and issue a Medical Certificate, if any, for PWD ID application from the local gov't
• Has parent interview and child observation
• Recommendations for therapy will also be given
• Appointment wait time: between 6 to 12 months.

✔️ Rate: Php 4,000 to 8,000+++

In this form, we will ask you to complete payment and submit your proof of payment.

Availability for Screening Session slots can take place between 1-3 weeks.  Please ensure that you have enough time to wait for a slot.

Once payment is received, all requests to cancel will be changed a cancellation fee.

OCCUPATIONAL THERAPIST

If your concerns are related to speech, with behavioral concerns

Examples of behavioral concerns:

• uncontrollable tantrums or behaviors
• social disinterest
• hyperactivity
• problems with focusing
• uncontrollable angry outbursts
• excessive fixations (e.g. on rotating objects, etc.)
• tip-toeing while walking
• no eye contact

SPEECH THERAPIST

If your concerns are purely related to speech, language and communication, without behavioral concerns (see above examples).

PHYSICAL THERAPIST

If you concerns are related to gross-motor skills or lack thereof.

Examples:

• balance is off
• not walking properly
• inability to "move" without assistance (e.g. sitting up, walking straight, and other bodily movements
• excessive clumsiness (e.g. bumping into objects/furnitures, falling off chairs, etc.) 

Policies

Payment is required in advance before we process your request to be booked for Screening.  Please ensure that you complete this form in full and ensure that you click "Submit".

You may reschedule your appointment at least 1-day ahead of schedule.  A Re-Booking Fee of P420.00 will be charged.

You may change your mind about proceeding with the booking but a cancellation fee of P616.00 will be charged.

Strictly no refunds for NO SHOWS or cancellations of less than 5 hours before the scheduled Screening Session.  Requests to change schedule less than 5 hours before your booked appointment will be considered a No-Show and no refunds will be issued.
