This request form will be submitted to the University of Toronto’s Freedom of Information and Protection of Privacy Office. Please enter either the 50% or remaining fee as quoted in your fee estimate letter.

Personal information provided in this form will be used for the administration of your FIPPA request only. Credit card information will be handled by the payment processor only and will not be collected or retained by the University of Toronto.
