I have read, understood, and agree to the North Shore Mi'kmaq Tribal Council Post-Secondary Education policies and guidelines as outlined in the NSMTC Student Handbook (All STUDENTS ARE REQUIRED TO READ THE NSMTC STUDENT HANDBOOK);

I agree to promptly report any changes to my enrollment status and/or program of studies;

I agree to promptly report voluntary withdrawal from studies as you will be held accountable for the potential loss of refundable tuition dollars and any living allowance paid beyond this point;

I agree to meet the standards required by both the educational institution and the North Shore Mi'kmaq Tribal Council Post-Secondary Education Program;

I agree to provide transcripts or statements of academic performance to the Post-Secondary Education Manager at the end of each semester in a timely manner;
*
I understand that any misrepresentation or omission of information is adequate cause for refusal or termination of funding by the North Shore Mi'kmaq Tribal Council;

I understand that if I am employed full-time, I am ineligible for the monthly student living allowance and any misrepresentation of this policy will result in termination of funding;

I understand that all necessary documentation as referred to on Page 5 of this form must be submitted to the NSMTC Post-Secondary Education department in order for my application to be considered complete, and that incomplete applications will not be considered.
