Child/Young Adult Care Services Sign-Up

We look forward to your attendance at our upcoming TANGO2 Family Conference in June 2024. Please fill out this form if you will be needing child/youth/young adult care services during the event--including individuals with TDD and their siblings.
Welcome to the Childcare/Teen/Young Adult Services for TANGO2 Research Foundation's upcoming family conference in Orlando, Florida!

We at Corporate Kids Events, Inc. (CKE) are very excited to be providing our care for your children. Our goal is to ensure the children have a safe, fun, and amazing time while you are enjoying your event. CKE has been in business for over 20 years and we take great pride in planning successful childcare for each of our clients.

Event Information

Event Date: June 24, 2024 and June 25, 2024
Event Times: 8AM until 5PM Daily
Event Room: TBD

Drop-Off Expectations
- The children will be grouped by age, as we have planned age-appropriate activities for each group.
- Please provide all infant and toddler needs, such as diapers, wipes, special foods, formula, bottles, cups, and extra clothes.
- Allow a few extra minutes for the first registration/drop-off… We require a short registration form to be filled out, and a security photo is taken for each family to ensure the safety of your child/children. Once we have your family registered, you will sign your child/ children into their age group and meet our wonderful staff.
- Each child will be given a wristband with their name, age and allergies to be worn while in our care. Parents will be given a ticket with the matching number on the wristband to be used at pickup. We ask that the ticket be provided for pick up, and if not available, the child will only be released to a parent or guardian in the security photo taken at registration.

In the event of an emergency, I hereby authorize any and all medical attention to be administered to my child (children) as is deemed necessary by an attending physician or nurse.

I understand and agree that I am financially responsible for any care so provided. In consideration of the opportunity to have my child (children) participate in the activities sponsored by Corporate Kids Events, Inc., I hereby assume all risks and waive all claims against the corporation, it’s respective officers, director, employees, agents and representatives for bodily injury, illness, or death and for damage to or loss of any property directly or indirectly arising from or in connection with any activities involving Corporate Kids Events, Inc. except to the extent directly and solely caused by the willful misconduct of the corporation or its agents.

I also understand and agree that management reserves the right to decline or discontinue enrollment based upon the management’s assessment of physical disabilities, illness, or medical conditions requiring an amount of attention or medical expertise beyond the company’s formal scope of ability.

Corporate Kids Events has my permission to take photos of my family and children at this event. Pictures may be used for digital photo CD and/or customer access via our website homepage and for client and promotion for future events.

Corporate Kids Events has my permission to take my child from the childcare room with supervision to use the bathroom, participate in a group game, or take a walk in the hotel, or convention center.
