This is a precautionary measure to assist us in following your wishes in the unlikely event your pet passes away while in our care. You can provide us with an emergency contact who will assume responsibility to pick up your loved one and follow through on your arrangements or you can give Silver Paws Lodge the responsibility of making arrangements for your pet at your veterinarian's office and we will arrange transportation there and pick up ashes for you if you choose to have them returned.

Every attempt will be made to reach myself and my emergency contact. However, if myself and/or emergency contact is unreachable, I understand that Silver Paws Lodge will need to transport my pet to my veterinarian's office on my behalf. Some veterinarian clinics require cremation/disposal fees to be paid on arrival.  If Silver Paws pays those fees on my behalf, I agree to reimburse the invoice provided by the clinic. 

By signing below you understand Silver Paws Lodge Rainbow Protocol and that there are fees associated with this. You give Silver Paws Lodge permission to follow your directions as stated above.
