-A copy of the original complaint or petition in the matter (from your jurisdiction)

-The foreign subpoena (from your jurisdiction)

-Any attachments that go along with the subpoena

-A copy of the case history or docket entries (a printout from the court showing the entries will suffice)

-A word document titled "Counsel of Record" (this should list the counsel for all parties to the case)

MJPS, LLC d/b/a LawServePro receives this request for service with the true understanding that it is for legal and lawful purposes and will not be held responsible or liable for any and all damages, claims and/or legal actions that may result in carrying out this request. Furthermore, we do not guarantee service will be successful. Once a request has been made, service is deemed in effect and therefore billable. We only provide services once payment arrangements have been made, we accept all forms of credit card payment, Visa, Mastercard, Discover and American Express. There are NO REFUNDS and payments made by credit card are non-refundable and uncontestable, and we reserve the right to refuse service to anyone for any lawful reason. If you need to cancel service after submitting your order, you may do so by emailing our service team at: service@lawservepro.com. There are no refunds when cancelling an order after submission. All of our service levels include up to three attempts unless the address is found to be a bad address (ie. the recipient moved, recipient is deceased, new tenants, bad address, etc.) in which case service will discontinue and a member of our staff will contact you to discuss options available. Upon successful service, we will provide you with a completed affidavit of service via the method you have indicated in the order form. The affidavit will need to be submitted to the court by you unless you select the affidavit filing submission add on in the order form indicating that you would like us to submit it on your behalf with the court. For subpoena domestication assignments, we do not guarantee when domestication will be completed. There is a collection fee of 35% for a delinquent credit card charge or past due amount with accrued annual interest of 8%.
