Player Registration Fees
Player registration fees help cover yearly helmet reconditioning, equipment replacement & repair, new game balls, replacement game jerseys & pants, game & practice field maintenance and painting, facility maintenance, athletic apparel (spirit packs), travel expenses, AHSAA Referees, HEMSI, athletic trainer supplies, laundry & cleaning supplies, Football Banquet, Middle School and High School pre-game player meals, and High School post-game player meals away games.

Fundraising
Each family is expected to support the football program by participating in program fundraisers. Or if fundraising is not for you, we offer a fundraiser buyout for $400.

Work Duty
Each family is expected to support the football program by signing up for a total of 4 pre-game, game-day, or field painting work-duty spots. If work-duty is not for you, we offer a work-duty buyout for $400.

Refund Policy

By checking 'I Agree' below and submitting this form, you, the Parent or Guardian of the above Player, acknowledge and agree to the Refund Policy. Unless it is a special circumstance, there will be no refunds of registration fees. A player who decides they would rather not play football after signing up is not eligible for a refund - Sorry. Any refunds will be a partial amount of the registration fee and will be determined on a case-by-case basis depending on what gear has been ordered for the player. Once football games start, no refunds will be honored.

By checking 'I Agree' below and submitting this form, you, the Parent or Guardian of the above Player, agree to release "Madison Academy Football", including Coaches and Assistant Coaches from liability for any and all injuries that may occur while training, practicing, performing, or during any football event or activity. You also agree that you are responsible for health and accident insurance and any medical costs incurred due to injury. You give permission for emergency medical transportation and treatment of your Player at your expense should the need arise. You give permission for the public display of any football images that your child may appear in. You agree to support the football program through Game-Day Work-duty assignments and Fundraising unless you purchase a Game Sponsorship, a Work-duty buyout, or a Fundraising buyout.
