As a future lash extension artist participating in this class, I acknowledge and agree to the following:
1. Client Safety: I understand that the safety and well-being of my clients are of utmost importance. I will adhere to industry best practices and standards for lash extension application, including proper sanitation, hygiene, and use of high-quality products.
2. Procedure Risks: I acknowledge that lash extension procedures carry inherent risks, such as eye irritation, allergic reactions, and temporary or permanent loss of natural lashes. I will take all necessary precautions to minimize these risks, including conducting patch tests and assessing client suitability.
3. Continuing Education: I commit to ongoing learning and improvement of my lash extension skills. I will utilize the knowledge gained from this class to enhance my techniques and provide the best possible service to my clients.
4. Aftercare Education: I will educate my clients on proper aftercare procedures to maintain the longevity and health of their lash extensions. I understand that failure to do so may result in poor retention, discomfort, or adverse reactions for the client.
5. Client Consultation: Before each lash extension procedure, I will conduct a thorough consultation with the client to assess their needs, expectations, and any potential contraindications. I will obtain informed consent from the client prior to the procedure.
6. Professional Conduct: I agree to conduct myself in a professional manner at all times during the class. I will respect fellow artists, instructors, and class organizers, fostering a positive learning environment. Failing to behave in a professional manner will cause expulsion from the class with no refund.
7. Limitation of Liability: I understand and acknowledge that the instructor of this class is not responsible for the outcome of any lash extension procedures performed by me following this class. I accept full responsibility for the procedures I perform, including client satisfaction and any associated risks.
8. Certificate vs. License: I understand that upon successful completion of this class, I will receive a certificate of completion, not a license. I am aware that it is my responsibility to check with the regulatory bodies in my state regarding the requirements for performing lash extension services publicly. Student may not receive a certificate unless all class requisites required by instructor are completed.
9. Photos and Videos: I consent to the use of photos or videos taken during the class for educational or promotional purposes by the instructor or class organizer, while ensuring client confidentiality and privacy.
10. Time Consideration: I understand that the daily classes might extend to more than the planned 4 hours depending on the performance of the student or instructor.
11. Refund Policy: I understand that once payment is processed there is no refunds. If the class is canceled or student does not attend class the monies paid in this agreement will be used for another class date. Class may be rescheduled due to insufficient bookings as for this class requires at least 10 students to be completed. Money for paying students in this case can and will be used for the next class available date. After a class cancelation or a student not attending, both the instructor and student have 60 days to create a new class date. Monies can also be put towards any other Ilumi Lash Products as well.

By participating in this class, I acknowledge my responsibility to uphold the highest standards of safety, professionalism, and client care in my lash extension practice. I also agree that registration is not complete until I have completed the payment on the Ilumi Lashes website located under the registration form. If payment is not submitted 24 hours after signing and submitting this registration form your information will be deleted from our records.
