If we receive more than one order per organization per month, only the first order will be fulfilled and subsequent orders for that month will be cancelled. If you are unsure of your funding type, please reach out before ordering to cdc@act-ct.org. We cannot offer tracking and we cannot ship to a PO box since an adult signature is required for delivery. If your business is for-profit and outside the field of public health, you are NOT eligible for this program without prior approval from DPH. All orders will be fulfilled within 14 business days.
