I HEREBY ACKNOWLEDGE THAT THE TRANSPORTATION OF ANIMALS MAY BE DANGEROUS AND ASSUME ALL OF THE RISKS OF USING TRANSPORTATION SERVICES OFFERED BY WINNER CIRCLE TRUCKING, LLC. I understand that WINNER CIRCLE TRUCKING, LLC and/or their service providers will be providing transportation for my animal(s). I am aware of the possible risks and dangers, and I knowingly accept and assume all known and unknown risks related to the transportation of my animal(s) including all physical, mental, or health issues that may arise during or after my animal’s transportation or while under WINNER CIRCLE TRUCKING, LLC or its service providers’ care, such as stress, illness, injury, disability or death, and all property loss or theft.

I HEREBY WAIVE, RELEASE, AND DISCHARGE THE FOLLOWING PERSONS: WINNER CIRCLE TRUCKING, LLC, its service providers, employees, contractors and owners of the vehicles used to provide transportation services from any and all known and unknown risks related to the transportation of my animal(s) or my use of WINNER CIRCLE TRUCKING, LLC pet transportation services, including all physical, mental, or health issues that may arise during or after my animal’s transportation or while under WINNER CIRCLE’S TRUCKING, LLC or its service providers’ care, such as stress, illness, injury, disability or death, and all property loss or theft, whether or not arising from the negligence or fault of WINNER CIRCLE TRUCKING, LLC.

I HEREBY ACKNOWLEDGE that my animal(s): is/are in good health and are up to date on all age-appropriate vaccines, has/have not harmed or shown or threatening behavior (i.e. biting or attacking) towards any person or other animal. WINNER CIRCLE TRUCKING, LLC and/or its service providers may at any time return or stop the transport of an animal if he/she exhibits behavior which could be harmful to other animals or persons.

In the event of any illness or injury, I hereby CONSENT to whatever medical treatment or hospital care from a licensed veterinary physician and/or surgeon as deemed necessary for the safety and welfare of my pet. By signing this waiver, I understand that I will be responsible for the cost of any medical treatment or hospital care required for my animal(s) while they are in the care of WINNER CIRCLE TRUCKING, LLC and/or its service providers. Whenever possible, attempts will be made to contact the owner prior to taking any medical action.

WINNER CIRCLE TRUCKING, LLC has a *15-minute window to load - +$50 per additional half hour. This includes delays due to additional time for the horse to load AND/OR wait time for late arrival of the contact person at either pickup or dropoff. *-- Also Totes/saddles: $50 small / $100 large. Depends on available space. *Payment due 1 hour before delivery or cash before unloading.

Payment Info: You will receive a text link for the invoice at pickup. Payment options are: Credit/Debit card (+3.5%) on the invoice ACH (1% fee), Zelle, Venmo, Cash/cashiers check. Electronic payments are due an hour before delivery and cash/check before unloading.
