If you do not attach the documents now, you will have 5 school days to provide them. Instructions on how to provide the documents to CUSD will be in your email confirmation.

*If your child does not have any discipline, please request a statement from the school on letterhead.

Please understand CUSD does not request student records for Open Enrollment Request. It is the Parent/Legal Guardian's responsibility to provide the required documents for a non-enrolled CUSD student.
