Volunteer Agreement
This agreement is designed to help clarify the responsibilities of Cascade Women's Health (CWH) to the volunteer and the responsibilities of the volunteer to the CWH.
CWH commits to provide the following:
· Orientation and appropriate training.
· In-service meetings for the purposes of: a) ongoing training; b) explanation of new policies and procedures; c) exchanging ideas and discussing volunteer related issues and d) team-building and spiritual enrichment.
· Policy and procedure manual(s) and other related reading materials.
· Experienced staff for advice and consultation.
· On-going education both onsite and online.
· An opportunity to self-evaluate performance prior to an annual review.
The volunteer commits to the following:
· Complete compliance requirements annually and comply with all applicable CWH policies and procedures.
· Attend a minimum of two volunteer in-service meetings per year.
· Attend special training sessions (no more than one such additional day of training will be expected annually).
· Read materials provided and learn to use documents required for position.
· Be on time and present for the entire scheduled shift unless prearranged with supervisor.
· Adhere to the CWH Statement of Faith, Statement of Principles and Mission Statement.
· Contact your supervisor as quickly as possible if you are unable to work your scheduled shift. All changes to your schedule must be pre-approved by your supervisor.
· Maintain patient confidentiality as mandated by HIPAA.
· Conduct yourself in such a manner to reflect grace, respect, and concern for all patients and visitors.
· Commit to maintaining sexual purity, abstinence if single, and faithfulness to your spouse, if married.
