  • North Peace Ball Hockey asks that all parents take an active role in their child's ball hockey experience. We require that you register to take part in at least one game this season in any one of the following roles:

    Bench Coach

    The Bench Coach is the primary designated coach for a Youth team for the duration of the 2024 season. This role may be filled in for by an alternate coach who registered for this role.
    Bench Helper The Bench Helper assists the team on the bench by opening the door, ensuring the players are staying hydrated, and assisting the Bench Coach.
    Team Mom The Team Mom is responsible for distributing and collecting jerseys before and after each game. She keeps team morale high by cheering on the players from the stands.

     

  • Note: There can only be one Bench Coach officially designated per team. Alternate coaches will be asked to fill in by the team on an as-needed basis.

    For safety reasons, Bench Coaches must submit to a criminal record check by the RCMP.

  • Consent for Disclosure of Criminal Record Information

  • To participate in the League as a youth team official, you must consent to the disclosure of your criminal record information (Form RCMP-GRC-3584e). If you have received a disclosure within the last 18 months, you may upload it below.

  • Please download and complete a Request for Form RCMP-GRC-3584e then submit it to the RCMP. Once it has been returned to you, you must email it to the League Commissioner before 10 March, 2024.

  • RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS & INDEMNITY AGREEMENT

    This Release of Liability, Waiver of Claims, Assumption of Risks & Indemnity Agreement ("Agreement") and releases the following facilities and entities (“Facilities”) from certain liability:

    Dave Barr Arena
    Sexsmith Arena
    North Peace Ball Hockey
    Wild Rose Ball Hockey Association
    Canadian Ball Hockey Association

    By executing this document, you acknowledge and waive certain rights, including the right to sue. Please review the contents carefully.

    I, being of the age of 18 years or older, hereby agree that, in consideration of being granted permission to enter and utilize the land and premises of the facilities listed above, which are leased to and operated by North Peace Ball Hockey, I, on behalf of myself, my heirs, successors and assigns, remise, release, indemnify, discharge, and forever hold harmless the Facilities, operators aforementioned, North Peace Ball Hockey, and other directors, employees, volunteers, coaches, instructors, agents, and independent contractors associated with the Facilities, from any claims arising due to disease, deterioration of health, illness, or injury to my person, including death, or for damages to or loss of my property resulting from my presence on and use of the lands and premises, and from accepting advice or due to the negligence of the operators listed above.

    In addition to be above, I understand that photographs of my person may be taken at all sporting events for use on North Peace Ball Hockey’s website, and in press, marketing materials, and other publications. By signing this document, I consent to the photographing and use my image and likeness.
