  • When to Use This Incident Report Form

    This form should be completed by Match Officials whenever an incident occurs at a match. It is used to report a range of issues that may impact the game. Please use the form to report any of the following:

    • Incident Reports – Including verbal abuse, physical altercations, or player/official injuries.
    • Ground Conditions – Such as weather-affected pitches, unsafe or unsuitable fields, or field marking problems.
    • Match Abandoned – Where a match is not completed due to reasons like team no-shows, severe weather, or spectator interference.
    • Other Issues – Including kit clashes, match sheet issues (players missing, etc.), or delayed kick-offs.

    If you would like wellbeing support or to talk to someone in relation to any issues that arise from submitting your report, you can request contact from a member of the Referees' Standing Advisory Committee (RSAC) by selecting the appropriate option. A member of the RSAC will then get in touch via email to arrange a chat through Microsoft Teams.
