School Registration
Use this form to Register multiple students for the Youth Art Show and Sale.
Please use a separate form for each division (Division 1, grades 10-12; Division 2, grades 7-9).
If more pages are needed, simply fill out another form with the division and teacher and school names. It is not necessary to re-enter the other information.
You will still need to complete the other required forms in order to enter the Show.
