    If you are wanting to register your family for Family Camp 2025, you are in the right place. This is our 3rd Annual Family Camp and we are so excited to continue this journey with you. We are welcoming all families who are members of our Nevada Chapter bleeding disorders community. 

    Cost: There is a $50.00 non-refundable administrative and amenity fee per family registration. If you are currently unable to pay the $50.00, you can request a fee waiver through this form.

    Transportation: Families are responsible for providing their own transportation to and from camp. It is approximately a 4.5 hour drive from Las Vegas, and 8.5 hours from Reno/Carson City area.

    Travel Reimbursement: Families will receive a travel reimbursement card/check per car at the closing of camp.

    Covid Health Notice: Covid vaccinations will NOT be required. For the safety of all campers and staff, all attending family members, ages 5 and up, will be required to submit to a Covid test upon arrival. It is recommended that families test independently at least 24 hours prior to attending to ensure a negative test, and also not attend if a family member is symptomatic. 

    Accommodations: Multi-family cabin-style bunk living, with private bathrooms. Each attendee will have their own bunk, families will be housed together. Must provide your own bed linens for bunk mattress. 

    Health Forms: All family members are required to complete a general health form to ensure the utmost safety and care is provided by our health team. Forms will be provided after registration is received.

    This year we invite our family campers to experience the excitement of the Amazing Race All-Camp Activities for a sense of community and adventure.

    We also have plans for your family to:
    - Dive into refreshing pool time
                     -Try their hand at archery and showcase your skills                   -Explore the great outdoors through forest trails
    - Engage in Camper's Choice Workshops tailored to their interests
    - Unwind with Movie Night under the starry sky
    -Enjoy a guided stargazing session
    - Satisfying full meals provided throughout your stay
    - Indulge your sweet tooth at the delightful Ice Cream Social and S'mores Challenge

  • Infusion Instruction Consent

    INFUSION INSTRUCTION CONSENT At camp, your child(ren) will have the opportunity to learn self-infusion instruction on a voluntary, informal, and individual basis by trained medical staff. Your child(ren) could receive this important training when they need factor replacement during camp, but only if the child is voluntarily ready to infuse themself or sibling. This training is also available to children who may not need to infuse for medical necessity.

    Please list the names of two people that will not be attending camp as emergency contacts.
  • MEDIA/PHOTO PERMISSION AGREEMENT

  • During programs or events sponsored by the NV Chapter-National Bleeding Disorder Foundation (NV-NBDF), you may take photos and or videos of me (us) and my (our) child(ren) that may be used in publications, websites and promotional materials which promote the NV Chapter-National Bleeding Disorder Foundation (NV-NBDF) activities. We may share photos with grantors not for public use. 

  • COVID-SAFETY AGREEMENT

  • COVID-safety protocols: It is our goal to provide the most welcoming and safe experience for all. Please note for our in-person events we will be following the most current COVID-safety protocols.

    In the event that someone tests positive upon arrival they will not be admitted to camp. If an attendee becomes symptomatic and tests positive during camp, active quarantine measures will be put in place until a departure can be arranged. 

  • PARENT/GUARDIAN SUPERVISION REQUIREMENT

    Please read and review the following.
  • Parent/Caregivers are to accompany their minor child(ren) at all times when they are not signed in to any kid/teen scheduled programming or sessions.

    NV Chapter-National Bleeding Disorder Foundation (NV-NBDF) will not be providing any form of childcare services or chaperones outside of any youth programming sessions and that it remains the responsibility of the parent/guardian to directly supervise their minor child(ren) at those times, including family activities. 

  • Family Camp Registration Fee

    There is a fee of $50.00 per family. This fee covers amenity and administrative costs for your family to attend camp.
  • Travel Reimbursement

    As families are required to provide their own transportation, we will be providing each family a travel reimbursement card/check to support their travel and gas expenses.
  • Fee Waiver

    We are aware that the cost of $50.00 may create an undue financial burden on some families. If you are currently unable to cover the cost of $50.00, please initiate the Fee Waiver to complete your registration. We will be in contact with you after we receive your registration.
  • Cancellation Policy

    We understand that cancelations do happen. Please be in contact with the NV Chapter if for any reason you are unable to attend so that we can make adjustments to our planning and open those spaces to families on our wait list. The $50 Family Camp fee is to cover administrative and amenity costs per family. Please note that this fee is non-refundable if a cancellation by email is not received before 5pm on June 6, 2025.
