TAX PREPARATION CHECKLIST
Please provide the following documentation:
• Income Documents:
- All Forms W-2 (wages)
- 1099-INT (interest)
- 1099-DIV (dividends)
- 1099-B (proceeds from broker or barter transactions)
- 1099-R (pensions and IRA distributions)
- Any other income reporting statements, including all copies provided by the payer
• Health Insurance Forms:
- Form 1095-A (for health insurance purchased through a public exchange)
- Form 1095-B (for health insurance purchased outside of a public exchange)
- Form 1095-C (for employer-provided health insurance coverage)
• Prior Year Tax Returns:
- If you are a new client, please provide copies of your last year’s tax returns.
• Mileage Records:
- Total mileage, commuting mileage, and business mileage for any automobile expenses claimed.
• Estimated Tax Payments:
- Details of any estimated tax payments made.
• Business or Rental Activities:
- Income and deductions categorized on a separate sheet for business or rental activities.
• Itemized Deductions:
- List of itemized deductions, categorized separately for medical, taxes, interest, charitable contributions, and miscellaneous deductions.
• Charitable Contributions:
- Copy of all acknowledgment letters from charitable organizations for contributions made during the tax year, including tithes and offerings from a church or organization.
TAX CLIENT ENGAGEMENT LETTER
Thank you for choosing OmniQuest Services for your tax preparation needs. We appreciate your trust in our services. This engagement letter outlines our mutual responsibilities, and your signature(s) signifies your consent to these terms and your trust in OmniQuest Services for preparing your return.
SCOPE OF SERVICES
Our firm will prepare your federal and state tax returns for the tax year. As part of our service, we conduct a thorough review of your financial information to ensure an accurate and compliant filing. For returning clients, we update existing information and ask about any changes that may have occurred during the year.
We retain copies of your submitted documents for three years, in accordance with the federal statute of limitations. After this period, your files and documents will be securely destroyed. Please ensure that every applicable field is completed before we begin preparing your tax return. For example, if you're filing jointly, we need email addresses and phone numbers for BOTH spouses. This information is REQUIRED even if there have been no changes or if you have provided it previously.
CLIENT RESPONSIBILITIES
You agree to provide us with all necessary documents and information required to prepare your tax returns. If you receive additional information after we have begun working on your return, you must notify us immediately to ensure all relevant details are included. Your tax return will be prepared using the information you provide. Before signing, please review it carefully to ensure all details are accurate. Once you approve it, we will electronically file your return. You will receive copies of the completed return for your records. We recommend that you keep a personal copy of your tax return and any related documents for your records for at least seven years. A fee may apply if you request a copy from us in the future.
Your review and confirmation are essential, as our services do not include auditing, reviewing, or verification unless explicitly stated otherwise. You confirm that all expense and deduction amounts are accurate and that you have the necessary supporting written records. In the event of an audit, you are responsible for verifying the accuracy of the reported items and providing written records for all entries included on your return, as required by the IRS or state tax authorities.
If you need to file an extension, a $75 fee will apply. Please note that filing an extension only extends the time to file your return — it does not extend the time to pay any balance owed. If you owe taxes, interest and penalties may accrue if payment is not made by the original tax deadline.
Clients are responsible for ensuring their bank allows the direct deposit of joint refunds into an individual bank account. If you terminate this engagement before completion, you agree to pay a fee for work completed.
DUE DILIGENCE
In compliance with tax regulations, we may request additional information or clarification on certain items. It is essential that you respond promptly to any such requests to ensure accurate and timely filing.
PRIVACY AND CONFIDENTIALITY
We value your privacy. All information provided is confidential and will not be disclosed to third parties without your consent, except as required by law.
FEES AND PAYMENT
We value our relationship with our clients and are committed to providing the best service possible. As a taxpayer, you can choose to pay your tax preparation fees using cash, credit card, electronic payment, or directly from your refund. However, if you select to have the fees deducted from your refund and your refund is intercepted, YOU REMAIN RESPONSIBLE FOR PAYING THE TAX PREPARATION FEES.
Please be aware that our tax preparation fee does not include additional services such as responding to IRS notices, IRS correspondence, or audit representation. If business clients do not provide the appropriate financial statements, OmniQuest Services will charge for bookkeeping services. Fees must be paid before we file your tax return. If you decide to terminate our services before the work is completed, you will be required to pay for the work completed up until that point. An additional fee applies for late returns.
Our price is based on the amount of work performed and the complexity of your tax return. By signing your tax return, you agree to pay the tax preparation fees, regardless of whether your refund is released. If your refund is intercepted, we will send you an invoice. If payment is not received within 30 days, we will proceed with ACH processing or collections, if no bank account is provided.
Please note: The Department of Treasury’s Financial Management Services (FMS) can offset any unpaid federal or government debt against your refund without prior notice. Common reasons for refund offsets include past-due child support, debts owed to a federal or state agency, unemployment debts owed to the state, and/or past-due federal student loans.
COMMUNICATION
Effective communication is crucial. As part of this engagement, we may communicate with you via text and secured portal transmission. Please be aware that emails can be intercepted, read, disclosed, or otherwise accessed by unintended third parties, or may not be delivered to all intended recipients. Therefore, OmniQuest Services will not send any personal information via email. DO NOT email us any personal information (e.g., Social Security numbers, birthdates) or documents containing personal details.
You agree that we are not liable for any loss or damage to any person or entity resulting from the use of email transmissions, including any consequential, incidental, direct, indirect, or special damages, such as loss of revenue or profits, or disclosure of confidential information. If you have any questions or concerns throughout the process, please contact our office via phone, text, or by scheduling an appointment.
MISCELLANEOUS
Because we are a paperless firm, it is not our policy to provide you with a hard copy of your tax return. If you would prefer a hard copy of the return, we would be glad to print one for an additional assembling/printing fee of $25. After data entry has begun and the client chooses not to have OmniQuest Services finalize the return, the client will be liable for time and expenses as of the date of notification. The client will be responsible for paying OmniQuest Services within five days of receiving the invoice. All original documents will be returned to the client.
CLIENT ACKNOWLEDGMENT
By signing below, you acknowledge that you have read, understood, and accepted the obligations and responsibilities outlined in this engagement letter. For a joint return, both taxpayers must sign. Your signature also indicates your consent to proceed with our tax preparation services for the specified tax year.
ACH AUTHORIZATION FORM
I, hereby authorize OmniQuest Services LLC to initiate electronic debits to my bank account for the purpose of collecting payment for the services related to the preparation of my tax return. I understand and agree to the following terms:
Payment for Tax Preparation Services: I acknowledge that I am engaging OmniQuest Services LLC for the professional service of preparing my tax return. Payment is not contingent upon the approval or funding of my tax refund.
Billing Authorization: In the event that my tax refund is not approved and funded within 30 days through a bank product with a third-party bank, I authorize OmniQuest Services LLC to electronically debit my bank account for the full amount of the tax preparation fees as agreed upon. This debit will be initiated as soon as practicable after the 30-day period has lapsed.
Bank Account Information: I have provided accurate and current information regarding my bank account for the purpose of this electronic debit authorization.
Right to Revoke: I understand that I may revoke this authorization at any time by providing written notice to OmniQuest Services LLC. Such revocation will not affect any fees or charges that have already been incurred.
Record Keeping: OmniQuest Services LLC will retain a copy of this authorization form, and I will receive written notice of any changes in the payment amount or schedule at least 5 days prior to the initiation of any debits.
By signing below, I confirm that I have read and understand the terms of this ACH Authorization Form and agree to be bound by them.
CONSENT TO USE OF TAX RETURN INFORMATION
For the purpose of this consent form, “we,” “us,” and “our” mean OmniQuest Services LLC.
Federal law requires this consent form to be provided to you. Unless authorized by law, we cannot use your tax return information for purposes other than the preparation and filing of your tax return without your consent.
You are not required to complete this form to engage in our tax return preparation services. If we obtain your signature on this form by conditioning our tax preparation services on your consent, your consent will not be valid. Your consent is valid for the amount of time that you specify. If you do not specify the duration of your consent, your consent is valid for one year from the date of signature.
If you do not consent, you may still have your tax return prepared and electronically filed by us for a fee.
For your convenience, we have entered into an arrangement with third parties to provide qualifying taxpayers with the opportunity to apply for an Electronic Refund Disbursement Service and/or Loan product. To determine whether these products may be available to you, we will need to use your tax return information in order to calculate the amount of your anticipated refund.
If you would like us to use your tax return information to determine whether these products may be available to you while we prepare your return, please sign and date this consent to use your tax return information.
By signing below, you authorize us to use the information you provide to us during the preparation of your 2024 tax return to determine whether to present you with the opportunity to apply for these products and services.
If you believe your tax return information has been disclosed or misused in a manner unauthorized by law or without your permission, you may contact the Treasury Inspector General for Tax Administration (TIGTA) by telephone at1-800-366-4484, or by e-mail at complaints@tigta.treas.gov.
CONSENT TO DISCLOSURE OF TAX RETURN INFORMATION
For the purpose of this consent form, “we,” “us,” and “our” mean OmniQuest Services LLC.
Federal law requires this consent form to be provided to you. Unless authorized by law, we cannot disclose your tax return information to third parties for purposes other than the preparation and
filing of your tax return without your consent. If you consent to disclose your tax return information, Federal law may not protect your tax return information from further use or distribution.
You are not required to complete this form to engage in our tax preparation services. If we obtain your signature on this form conditioning our services on your consent, your consent will not be valid. If you agree to the disclosure of your tax return information, your consent is valid for the amount of time that you specify. If you do not specify the duration of your consent, your consent is valid for one year from the date of signature.
You have indicated that you are interested in receiving an Electronic Disbursement Service and/or Loan (or collectively, “Products or Services”) from Pathward, National Association. In order to provide you with the opportunity to apply for one of these Products or Services, we must disclose all of your 2024 tax return information necessary for evaluating the request to Pathward.
If you request a more limited disclosure of tax return information, you will not be eligible to submit an application request for these Products or Services. If you would like us to disclose your 2024 tax return information for this purpose, please sign and date your consent to the disclosure of your tax return information.
By signing below, you authorize us to disclose to Pathward all of your 2024 tax return information necessary for the evaluation and processing of your request for a Product or Service. You understand that if you are unwilling to authorize the disclosure and sharing of your tax return information with Pathward, you will not be able to obtain a Product or Service, but you may still choose to have your tax return prepared and filed by us for a fee.
