FULLTIME/ PART TIME PROGRAM REGISTRATION FORM

By signing this form, I give permission for my child’s photograph to be used for advertising and promotional purposes. I understand that no other information will be distributed. I understand that fees are due for classes and competitions provided and entered (including all missed lesson fees) and will finalise fees prior to the end of the third week of each term. Any fees that are not paid 2 weeks after receiving them will receive a 10% late payment charge. Any student who has not made full payment of fees after 3 weeks will not be able to participate in classes until fees are paid. I understand that if I decided for my child to stop attending Ablaze Dance Academy, I will be required to give 2 weeks’ notice and pay for the following 2 weeks’ worth of classes. *No student will be able to re-enrol for the classes until overdue fees are paid. If accounts are forwarded to a debt collector, it will be done at the expense of myself (the fee payer). I understand that there is a possibility of physical injury by participating in dance classes and performances, and that if required teachers or coaches will call emergency assistance if needed and that any expenses of such are my responsibility. Students behaving in an unsafe manner will be asked to sit out until their behaviour is appropriate.
