FINANCIAL AGREEMENT & ENROLLMENT CONTRACT

1. GDEI Standard Plan

Tuition is due the day of class, after class finishes. For all payments directly to GDEI, we accept CASH, CHECK, VISA, DISCOVER, or MASTERCARD. Application Fee of $100 may not be financed. A Co-Signer Agreement is required for all students paying GDEI directly. An Auto-Debit Contract is also required for all students paying GDEI directly. Each student must make payments according to their chosen plan. By signing the enrollment agreement, the student agrees to follow the payment plan below and adhere to the financial policy of GDEI.

2. CareCredit and Other Financing Options

CareCredit is a credit card that provides up to 24 months at 0% interest. If you enroll in with this option there is a financing charge added to the tuition depending on what term you select. Acceptance to this program is pendingcredit approval through student loan company, a third party, and having a cosigner is recommended. Application Fee of $100 can be financed (table below assumes this).

Your tuition payments must be made on the due dates according to your schedule, otherwise you will incur Late Fees. If you are paying by credit card, call the school to process your payment. If you are paying by check, please leave your check with the secretary at the school or mail it to the address above.Cash and Money Orders are accepted as well. You are responsible for making on time payments and keeping current with your balance. Listed below is our late/no payment policy as described in our course catalog and enrollment agreement:

Late Fees 1) All delinquent accounts will incure a $10/day late fee. 2) Student will NOT be permitted to return to class until current with payments (including Late Fees). 3) Late Fees may NOT be financed. 4) A student's account that is delinquent (where balance is not paid up to date) for more than 30 days will be sent to our Collection's Agency for recovery and the student will forfeit their Dental Assisting Certificate. The student is responsible for any and all bank fees related to a check that is returned for insufficient funds. The student is responsible for arranging on time payment if a credit card is declined. GDEI Auto-Debit Contract It is mandatory that every student on the Standard GDEI plan sign the Auto-Debit Contract. This option allows the student to have tuition payments automatically taken from their credit card account on the day the payment is due. This option eliminates the worry of any Late Fees being incurred, as long as the financing account has appropriate funds.

Co-signer Agreement - Every student on the GDEI Standard Plan must have a Co-signer Agreement completed prior to acceptance into the school. This agreement is to ensure tuition payments are kept current, otherwise the student’s co-signer will be required to make the student’s payments.)Payment Plan for Tuition (Co-signer is required with GDEI Standard Plan) GDEI Payment Plan OR Financing

Financial Responsibility - I understand that my total account balance is my responsibility, regardless of the financing option I have chosen.I understand that my account will be sent to a collections agency, I risk forfeiting my Dental Assisting Certificate, and I will not be allowed to return to class if I do not abide by the terms set in my contract with Georgia Dental Educational Institute.

Postponement of Starting Date- Whether at the request of GDEI or the student, a written agreement signed by the student and GDEI is required. The agreement must set forth: a. Whether the postponement is for the convenience of GDEI or the student and: b. A deadline for the new start date, beyond which the start date will not be postponed. If the course is not commenced, or the student fails to attend by the start date set forth in the agreement, the student will be entitled to an appropriate refund of prepaid tuition and fees within 30 days of the deadline of the new start date set forth in the agreement, determined in accordance with GDEI’s refund policy and all applicable laws and rules concerning the Private Occupational Act of 1981 Complaints Student complaints should be brought to the attention of the director to attempt resolution. The director and student are to follow the grievance procedures according to school policy printed in the school catalog. If satisfactory resolution cannot be reached between the student and the school, the student may file a written complaint online with the appropriate contact as indicated in the course catalog.

By signing below, the student agrees to pay the Georgia Dental Educational Institute (GDEI) the total stated tuition and fees according to the financial plan I have chosen. The school agrees to provide the occupational training in accordance with the provisions of the school’s current Catalog. Payment of all monies due shall be a condition of continuing enrollment. Student agrees to adhere to all attendance and conduct policies as stated in the school’s current Catalog. Upon satisfactory completion of all academic and skill requirements and when all financial obligations to GDEI have been met, GDEI will award the Certificate to the student. The student and GDEI understand that this Enrollment Agreement, may not be amended except in writing and signed by both parties.

I HAVE RECEIVED, UNDERSTOOD, AND AGREE TO THIS COPY OF THE ENROLLMENT AGREEMENT AND THE CURRENT SCHOOL CATALOG. IF I PLAN TO MAKE PAYMENTS BY CREDIT CARD, I AUTHORIZE THE GEORGIA DENTAL EDUCATION INSTITUTE TO KEEP MY SIGNATURE ON FILE AND TO USE THE CREDIT CARD INFORMATION I PROVIDED FOR PAYMENTS ACCORDING TO THE AGREED UPON PAYMENT SCHEDULE UNTIL MY BALANCE IS PAID IN FULL. REFUND POLICY Any student may cancel the enrollment agreement or contract by written notice without any penalty or obligation and receive a full refund of any monies paid within 3 days after the enrollment agreement or contract is signed and a tour of the facilities is made. This excludes Saturdays, Sundays and Legal Holidays. The enrollment agreement is not binding until signed by the School Director.
