• Cleveland Community Cat Project Trap-Neuter Request Form

If you are requesting assistance with community cats, please complete this form in its entirety. This helps us streamline requests. Due to the volume of requests that we receive, we are not able to respond to incomplete forms, nor are we able to respond to requests by email or phone that do not accompany a form. This form is not meant to be used for true emergencies requiring immediate intervention. If a cat is having an emergency, please take it to an emergency veterinary clinic or contact your local animal control.

  • Please complete the fields below for each cat at this location.

    • Yes, that means you need to list ALL known cats at this location.
    • Do not submit this form for a specific cat when there are six other cats there. We have to know how many cats there are because it's often impossible to trap a specific cat.
    • View this Kitten Progression resource to differentiate between neonates (kittens under 8 weeks) and juveniles.
    • Only check "Friendly" if the cat is approachable, and you have personally handled or pet the cat.

  • Do you feed or otherwise care for these cats regularly?*
  • Are any of the cats you are requesting help with already on a feeding schedule (i.e., you put food out at the same time each day, and they reliably show up to eat)?*
  • Do these cats have winterized cat houses or shelter from the elements?*
  • How often do you see these cats?*
  • Cleveland Community Cat Project is an all-volunteer organization with no physical shelter. Everyone who helps—every trapper, every foster, every transporter—has a full-time job, other pets, and limited space. We prioritize requests where the caregiver is willing to participate.

  • Please select all the ways you are able to assist. The more you can participate, the faster we can help. If you cannot assist with any of the listed tasks, we may not be able to accept your request.*
  • Are you able to donate toward the veterinary costs for the cat(s) you are requesting assistance with?*
  • You will receive a confirmation email after submitting this form. Thank you for caring for community cats.
