WAIVER AND RELEASE OF LIABILITY
Impact Martial Arts LLC (referred to as the 'school') urges you and all students to obtain a physical examination from a doctor before using any exercise equipment or participating in any exercise class. All exercises, including the use of weights and use of any and all machinery, equipment, and apparatus designed for exercising shall be at the student’s sole risk. Student understands that the agreement to use, or selection of exercise programs, methods and types of equipment shall be student’s entire responsibility, and the School shall not be liable to student for any claims, demands, injuries, damages, or actions arising due to injury to student’s person or property arising out of or in connection with the use by students of the services, facilities, and premises of the School. Student hereby holds the School, its officers, owners, agents and employees harmless from all claims which may be brought against them by student or on student’s behalf for any such injuries or claims.
SCHOOL RULES
Undersigned gives full consent to the school to make any modifications in the class schedule as deemed necessary or to close the school on national holidays, for the purpose of special events, weather and a period not exceeding 2 weeks consecutively per calendar year. Student agrees that any and all photo(s) and/or video(s) in possession of the school is the sole property of the school and may be used at the schools discretion. Student further agrees to act responsibly with the techniques that will be taught at the school for self defense. Student understands that these techniques can cause harm and under no circumstances shall the information learned here be used outside thee school unless required in self defense. Student pledges at all times to act in a manner becoming a martial artist and to uphold the time honored traditions and Tenets of Taekwondo. Failure to do so may result in reprimand or dismissal from the school
CANCELATION POLICY
With our month-to-month membership, you have the flexibility to cancel or pause your Impact Martial Arts membership at any time by providing us with just 30 days’ written notice. To initiate the cancellation process, simply sign the cancellation form at the school. The termination will take effect on the 30th day after we receive your
notice, known as the “Termination Date.” Please note that any charges due before the Termination Date will still apply. For example, if you submit your notice on March 5th, the Termination Date will be April 4th, and any membership fee charges due between those dates will still be applicable. If you choose to cancel or pause your membership and return after, your membership tuition fee may change to the current membership fee. Example; if you are currently paying a discounted fee compared to the tuition fee at the return date, your tuition fee will match the returned date tuition.
