Important Intake Notice – Per Tax Year

This intake form is for the 2025 tax year being filed and is intended only for clients who plan to file their tax return with Prospered Taxes Enterprise LLC; if you do not intend to file with me for this tax year, please do not complete this form. By submitting this intake, you authorize document review, discussion of your return or potential refund, evaluation of advance options (if applicable), and preparation of documents for signature. A $25.00 non-refundable document review fee applies per tax year, which includes review of uploaded documents, determination of service scope, and identification of missing items; this fee applies whether or not you proceed, and if you move forward with tax preparation, it will be credited toward your tax preparation fee.
