Rules: canine success factors

1. Please try to leave at least 6 feet between your dog and another dog when entering and exiting the facility/classroom. Some dogs are shy and others will be excitable, we want to make certain no dog feels overwhelmed or fearful by an unexpected greeting.
2. Bring more treats than you will need, and try to bring a few different types to keep your dog interested.
3. If your pup has an accident, no problem! We have cleaning supplies on hand. However, each family is responsible for cleaning up after their dog(s) both inside the facility and outside.
4. All family members are very much welcome to attend! In fact, we find that it's best to have anyone who will be working with your dog present so that you can maintain a consistent training plan. However, children must practice the same etiquette they would in a school classroom. At the end of the day, it's important to remember that this class is for your dogs!
5. For the protection and safety of our canine students, those who fail to provide current proof of vaccinations and a fecal exam at least five days before the start date, will not be permitted to attend a group lesson. You may send an electronic copy to godogsoviedo@gmail.com. If you are already a client at Shaffer Animal Hospital and you have not already been given confirmation to attend, please contact us at 407-366-6738 so that we can check your vaccination history. Required Vaccinations: Rabies, DHPP, Bordetella, Canine Influenza (H3N2), and a parasite-free fecal exam. If your dog is under 6 months of age then he or she will not have completed the full course of vaccines. You are still permitted to attend as long as your puppy has the required vaccines for his or her age. If you have any questions about that, please discuss with your veterinarian or send us a message.
6. Payment is due no later than the Thursday before the first class. You can stop in to visit us at G.O.D.O.G.S. Monday through Thursday from 8:30 am to 5:30 pm to pay in person or you can call us at 407-366-1722 to complete the transaction over the phone. Please NOTE that until payment is received, you will not have a secure spot in the class.
7. We understand there may be unforeseen circumstances that may cause you to not be present for a class. You will have access to all the homework assignments for the duration of class should you miss a day. All learning material is proprietary and you will no longer have access after the course. Refunds are not permitted for missed classes.
8. You agree to the following & understand that failure to abide by the class rules may lead to suspension/dismissal from class.

• I will NOT allow my dog to greet other dogs on leash.
• I will NOT remove my dog’s leash at any time unless given express permission to do so by the Instructor.
• I will NOT let go of my dog’s leash at any time unless given express permission to do so by the Instructor.
• I will NOT let my dog run up to another dog on leash at any time unless given express permission to do so by the Instructor.

These Terms and Conditions are by and between the Owner listed on the first page of the Agreement (“Owner”) and Shaffer Animal Hospital/ GODOGS (“SAH/G”). The first page of this Agreement is hereby included by reference. These terms and conditions are applicable to the Owner and any pets listed on the first page of the Agreement and shall remain in full force and effect for all of Owner’s pets that receive services hereinafter whether individually listed or not (“Pets”).

Owner specifically represents that he/she is 18 years of age or older and is the lawful and valid owner of the Pets. Payment for services is due before the training sessions begin. Owner agrees to pay the rate as set forth. Owner agrees, understands, and acknowledges that each dogs ability, breeding, and temperament is different and the elimination or modification of behaviors is not guaranteed and dog training will not provide exact results. Owner represents all Pets to be at least 2 months of age.

Owner warrants and represents that Pets are not show pets or working pets and Owner does not generate revenue from the Pets now or anticipate such in the future.

SAH/G shall exercise reasonable care for Pet. However, due to the social nature of a training facility, there are some inherent risks in training Pets. These risks may include, but are not limited to, transfer of communicable diseases such as Canine Papilloma virus (Puppy Warts), Kennel Cough, Diarrhea (Stress Colitis), and Canine Upper Respiratory Infection and minor injuries including, but not limited to, broken nails, sore feet, puncture wounds, abrasions and cuts. These injuries are generally benign and can usually be managed by our staff or at home. In some cases, veterinary care may be required.

It is expressly agreed by Owner and SAH/G that SAH/G’s liability, if any, shall not exceed fair market value for the Pets or the total training fees paid to SAH/G for the most-recent training session, whichever is less.

Owner agrees that he/she and family may be exposed to inherent risks during training and assumes all risks of injury from Pet. Owner agrees to release, indemnify, and hold harmless SAH/G from any and all manner of damages, claims, loss, liabilities, costs or expenses, including reasonable attorney’s fees and related costs, arising out of or related to SAH/G’s services, including, without limitation, (i) any inaccuracy in any statement made by yourself or information provided by you to SAH/G (ii) your Pet, including but not limited to destruction of property, dog bites, injuries, and transmission of disease, and (iii) any action by Owner which is in breach of the terms and conditions of this agreement. The Owner further agrees to be solely responsible for any and all acts or behavior of Pets while on the premises for SAH/G classes. Owner agrees to be financially responsible for SAH/G property damaged by Pet.

Owner agrees that in the event the training charges are not paid when due in accordance with this agreement, SAH/G may charge interest at the rate of 1.5% per month until paid in full. In the event legal action is taken to enforce the terms of this agreement Owner agrees to pay SAH/G’s court costs and reasonable attorney’s fees.

This Agreement contains the entire agreement between the parties. This Agreement shall be governed by and construed in accordance with the laws of the state in which SAH/G is physically located and each party consents to the exclusive jurisdiction of the said state. Furthermore, the parties agree that any legal actions shall be subject to the venue of the court of competent jurisdiction in the county in which SAH/G is physically located.

Owner authorizes Shaffer Animal Hospital/ GODOGS to use pictures of Pets taken during the obedience training classes on its website and in other promotional materials.

By signing this Agreement, Owner certifies to the accuracy of all information given about Pet to the best of his/her knowledge.
