Tattoo Booking Form

Hello! Thank you so much for your booking inquiry:) Please read through the entire form carefully and fill out each question. Please allow 24-72 hours to process your request. You will get an email response from chadwicktattoos@gmail.com. If you have any further questions or concerns feel free to reach out to me through email. Please keep in mind I do not copy any work. Please read carefully as the information below is important for the booking process.(I know it's long, please forgive me) At the bottom of this there will be a form that you will fill out. If it is not filled out properly, I will resend the form, so please make sure to fill out everything correctly! I specialize in Black and Grey and Anime. A deposit will be required to book. The deposit is $180 down which will come off the total price of the tattoo! CASH IS Preferred day of appointment but not required. I will require the deposit the day you set the appointment, or within 24 hours of setting the appointment, or your spot will be given to someone else! I will be allowing a given reschedule, but be aware that any reschedule after that may result in forfeit of your appointment and deposit. DEPOSITS ARE NON REFUNDABLE/TRANSFERABLE!!! If you book multiple sessions you will be required to pay a 180$ deposit for each required session. If you do not give me any notice and NO CALL NO SHOW your appointment you will be expected to pay a new deposit along with a additional 180$ NO SHOW fee before you are allowed to rebook. POLICIES-You must be 18 years or older to get tattooed.-You must present your ID the day of your appointment or YOU WILL NOT GET TATTOOED. - NO CHILDREN- NO SPECTATORS unless previously discussed with the artist.-If you are sick please reschedule ahead of time.-I require AT LEAST a 48 hour notice for reschedules. If you reschedule past the mark, another deposit will be required.-You are allowed ONE reschedule -Please arrive on time to your appointment. If you are running late, please let me know ahead of time and I can work with you. If you arrive more than 15 minutes late without notice, your appointment will be considered a no call, no show and your appointment will have to be rescheduled, which would require another deposit. -If you do not show up to your appointment or arrive too late, your deposit will be forfeited, and can not be returned or used for another appointment.-If you book a large tattoo and for some reason you can not sit for it, you are still expected to pay the remainder that is left, and you must pay an additional rate to finish the piece.- Please make sure to arrive fed and hydrated for an enjoyable tattoo experience. CREATIVE FREEDOM- I want to create something just for you. I do not redo tattoos I have done in the past. Realistic tattoos usually require reference images (rather than a drawing) but If the design dictates that a drawing be produced, a line drawing/sketch can be given the day of. With that being said, I am not able to show you the design prior to your appointment. Please trust that I will do the best I can for you. If you arrive and there are small changes that need to be made, of course I will work with you. Make sure once you fill out the form, all specifics in the design are addressed and clear to make sure I design exactly what you're looking for. Please know that any major changes made to the design the day of your appointment will result in a last minute reschedule in order for me to redraw the design for you.
