1) A complete waiver is required to play. NO exceptions!
2) No shoes allowed in playland area and socks are required at all times.
3) Grip socks are required to play in playland, regular socks are required for toddler area. Shoes are allowed in game room and party room only.
4) No outside food or beverages permitted. Food must remain in party room or Dino Snack Shack area.
5) Play equipment must be used as intended. Example- the slide is for going down & not up and on a child's bottom not stomach.
6) I agree to watch the minors in my charge and will assume responsibility for any damage done from misuse of equipment.
7) I agree to have the minors in my charge keep sensory items such as rice & water separate from each other and at the location they are found.
8) I understand that the equipment in the playland is for the use of children 10 & under and will be sure that no older children in attendance or adults in my party, will use any of said equipment. I agree to assume responsibility of any and all damage created from such actions.
In consideration of being allowed to enter the play area and game room, or to participate in any party and/or program, at BUILD-A-PET CLUBHOUSE, LLC located at 502 1st Ave, Sterling, IL 61081 (the "Premises"), the undersigned, on his or her behalf, and on behalf of the minor(s) identified below, acknowledges, appreciates, agrees and is fully aware of the risks and hazards in participating in any of the activities on the Premises and further agrees as follows:
I, for myself, my child or ward, sign this Acknowledgement of Risk/Waiver of Liability in consideration of the opportunity to use the facilities or to participate in any parties or activities at/by BUILD-A-PET CLUBHOUSE, LLC.
I, for myself, my child or ward, acknowledge and understand that there are dangers and risks associated with the activities at/by BUILD-A-PET CLUBHOUSE, LLC and agree to assume any and all risk of personal injury, including, but not limited to scrapes, bruises, broken bones, disabilities and other injuries, including the potential for paralysis and death
I, for myself, my child or ward, agree to follow the safety instructions provided and acknowledge that failure to do so may result in expulsion from BUILD-A-PET CLUBHOUSE, LLC.
I, for myself, my child or ward, and on behalf of my or their heirs, assigns, personal representatives and next of kin, HEREBY HOLD HARMLESS BUILD-A-PET CLUBHOUSE, LLC, its owner(s), member(s), officer(s), employee(s), director(s), agent(s), Landlord(s), trustee(s), equipment manufacturer(s) and sponsoring agencies from all liability for any such personal injury, disability, death or loss or damage to person or property, including any and all negligence claims by other participants, guests, and/or employees of BUILD-A- PET CLUBHOUSE, LLC, to the fullest extent of the law. By execution of this agreement, it is my intention to assume all risk of injury and do hereby surrender and waive any rights to sue or exercise any legal right to seek damages against BUILD-A-PET CLUBHOUSE, LLC, its owner(s), member(s), officer(s), employee(s), director(s), agent(s), Landlord (s), trustee(s), equipment manufacturer(s) and sponsoring agencies and all other persons acting on its behalf. I agree to reimburse all reasonable attorney's fees and costs incurred by BUILD-A-PET CLUBHOUSE, LLC d/b/a BUILD-A-PET CLUBHOUSE, in defense of any and all claims of negligence, liability, demands, actions or causes of action.
I understand that this document is a contract. I represent that I have read it thoroughly and I further represent that I fully understand the terms.
