1. Submit copy of Current City of Glendale Tax License. All vendors must obtain a business license with the City and State to participate in the City of Glendale events. Your license must be obtained prior to the event during regular business hours at the Tax & License office. You may obtain a Glendale Sales License or occasional sales permit (depending on your gross sales) by contacting the Tax & License Department at 623-930-3190 or in person at 5850 W Glendale Avenue, 1st floor.
2. Submit copy of Current Maricopa County Permit. All vendors with any type of food items must submit a temporary permit application and be pre-approved by the Maricopa County Health Department Special Events Program. Health Permits must be displayed in your booth throughout the event. For more information, call the Maricopa County Health Department at 602-506-6978, contact by email at specialevents@mail.maricopa.gov or visit the Special Events website at http://www.maricopa.gov/EnvHealth/SpecPro/SpecEvents.aspx. All pertinent laws, ordinances and regulations pertaining to health and public safety shall be strictly observed.
3. Submit copy of menu/price list. Vendors must provide prices for items being sold and no handwritten signs will be allowed. Vendors may sell only what has been agreed upon in the vendor contract. This may be all or a portion of your submitted menu items.
4. Submit photo of set-up. Professionalism is expected of all vendors. Vendors must be ready to sell 30 minutes prior to start of event and stay for the duration of event. Vendors are responsible for the disposal of their own refuse. Vendors must be self contained (provide thier own power, lights, etc.). Vendors may drive their vehicle to their designated vending location for unloading and loading. All vehicles not included in your application information must be parked in designated parking spaces 45 minutes prior to event start time.
5. Submit copy of Certificate of Insurance in the amount of 1,000,000 listing City of Glendale as an additional insured. As an approved vendor, you must provide the city with a valid Certificate of Insurance (COI). The COI should list the city of Glendale as an addtional insured for one million dollars per occurance, naming the specific event that you have been accepted for. (This is only applicable for the interactive, amusement and food vendors).

Fire Information:
The City of Glendale Fire Prevention Office requires all vendors that use flammable compressed gas to perform a leak check of all connections which needs to be witnessed by the onsite inspector. Each vendor is required to provide a spray bottle of commercial leak test solution or a spray bottle of soapy water to perform the test. If a leak is found, the leak must be repaired prior to the use of the propane container. If you are using any propane or open flame, please read fire code information. A fire extinguisher, minimum classification 2A10BC, shall be placed in all cooking tents. Where deep fat frying occurs, an additional Class K fire extinguisher is required. Any fire extinguisher used during events must have an inspection performed by a third party company and a tag attached and dated within the last year. Any food truck that performs food preperation and cooking shall maintain all required fire extinguishers and if cooking involves the use of deep fryers, grills and/or griddles shall have a commercial kitchen hood with appropriate fire suppression system that meets UL 300 listing. The commercial hood system shall be maintained and cleaned as needed with the fire suppression system being inspected and tagged by a competent third party on a 6 month schedule. There will be no exceptions to this requirement.
