Please use this form to make your reservations for the Annual Recognition Dinner hosted by the Eagle Foundation.
The RSVP Deadline is March 26. No refunds can be made for cancellations after March 31. This form can be used for making reservations for 1-12 guests. Or you can pick 0 guests if you are NOT attending but would like to make an honorary donation.
If you are part of a building or group and want to sit together, PLEASE SUBMIT ONE FORM for the entire group. Please use this form only to reserve seats. If you are already on the guest list, and only NEED TO PAY for your ticket, use this link so we avoid duplicate guest names.
The cost of each guest is $60.00 (the Eagle Foundation or school PTO will pick up the cost of all honorees and one guest).
If you register 1-4 persons, we will seat you at a table with other guests. If you are registering 5-12 persons, we will attempt to reserve a table just for your group, but there is a possibility others will be assigned to your table depending on space. Ten guests is an ideal number at a large table, 11 guests is tight and 12 is the max - no wiggle room.
