I hereby give my full consent and approval for my child to participate in The Milton Sports Hall of Fame Sports Camp. I understand that there are certain risks of injury inherent in the practice and play of the sports and other related activities incidental to my child’s participation, and I am willing to assume these risks on behalf of my child. I hereby certify that my child is fully capable of participating in this active sports camp and that my child is healthy and has no physical or mental disabilities or infirmities that would restrict full participation in this sports camp, except as listed in the medical information section. I understand that there will be no medical personnel on-site at the camp. I further understand that the camp personnel will not administer any medications (over the counter or prescription) to an athlete. In the event of any emergency, if I cannot be reached by phone, I give permission for camp personnel to exercise their discretion to have my child transported to hospital. In addition to giving my full consent for my child’s participaction, I do hereby waive, release and hold harmless The Milton Sports Hall of Fame, its officers, camp supervisors, volunteers and representatives for any injury that may be suffered by my child in the normal course of participation in the sports and activities incidental thereto, whether the result of negligence or any other cause. I permit photos and videos of my child to be used for general Milton Sports Hall of Fame promotions.
Refund Policy
No refund will be be provided if requested within 20 days of the start of the Camp/Clinic. Refunds will be issued if the request is received by the Camp/Clinic Director at least 21 days prior to the start of the Camp/Clinic. A 10% handling fee will be applied to eligible refunds. If we cancel a Camp/Clinic, we will issue a 100% refund. We follow public health guidelines and will refund 100% of the fee if public health measures cause us to close camp before the first day, or a prorated amount if camp is closed after the first day. In all cases, the credit card processing fee will not be refunded.

The Milton Sports Hall of Fame was Incorporated on March 10, 2016 under the Canada Not-For-Profit Corporation Act. The purpose of the Milton Sports Hall of Fame is to honour outstanding achievement in sports by athletes, sports teams and builders in our community. The Milton Sports Hall of Fame is located at the Milton Sports Centre.

The MILTON SPORTS HALL OF FAME is conducted 100% by volunteers from Milton and proceeds from this camp support the Induction Ceremony of the Class of 2023.
