INSTRUCTION FOR PARTICIPATION

1. Dress Code: Please wear comfortable athletic attire and non-marking athletic shoes suitable for indoor basketball activities.

2. Arrival Time: Participants should arrive at the venue at least 15 minutes before the scheduled start time to allow for check-in and pre-warmup routine.

3. Equipment: Extra Mile Basketball NJ LLC will provide basketballs for the training session. Participants are encouraged to bring their own water bottles, towels and extra pair of clothes, as needed.

4. Code of Conduct: All participants are expected to conduct themselves in a sportsmanlike manner, respecting fellow participants, instructors, and facility rules at all times.

5. Safety: Participants must follow all safety instructions provided by instructors. Report any injuries or concerns immediately to coaches and staff.

6. Photography and Video: Professional photographers/videographers may be present. If you do not wish to be photographed or filmed, please select "Non-Consent" in the Photo/Video Release Waiver below and please notify coaches and staff upon arrival.

7. No refunds: By registering, you will hold a spot, therefore do your best to show up or communicate with the head coach to reschedule.

8. Enjoy the Experience: Whether you plan to continue your lomg term basketball training with us or you are just checking us out this one time, have fun, learn, and make the most of this experience!

ACTIVITY LIABILITY WAIVER FORM
DETAILS OF ACTIVITY

1. As scheduled, the Participant will be participating in the following activity: A single 1 hour (60 minutes) tryout session provided by the Activity Provider.

CONSIDERATION

2. Being of lawful age and in consideration of being permitted to participate in the Activity, the Participant releases and forever discharges the Activity Provider, the Activity Provider's spouse, heirs, executors, administrators, legal representatives, and assigns from all manner of actions, causes of action, debts, accounts, bonds, contracts, claims, and demands for or by reason of any injury to person or property, including injury resulting in the death of the Participant, which has been or may be sustained as a consequence of the Participant's participation in the Activity, and not withstanding that such damage, loss, or injury may have been caused solely or partly by the negligence of the Activity Provider.

3. The Participant understands that the Participant would not be permitted to participate in the Activity unless the Participant signed this Waiver.

CONCURRENT RELEASE

4. The Participant acknowledges that this Waiver is given with the express intention of effecting the extinguishment of certain obligations owed to the Participant by the Activity Provider, and with the intention of binding the Participant's spouse, heirs, executors, administrators, legal representatives, and assigns.

FITNESS TO PARTICIPATE

5. The Participant acknowledges to the Activity Provider that the Participant does not have any physical limitations, medical ailments, or physical or mental disabilities that would limit or prevent the Participant from participating in the Activity. If required, the Participant will obtain a medical examination and clearance.

FULL AND FINAL SETTLEMENT

6. The Participant acknowledges and agrees with the Activity Provider that: (1) the Activity Provider has given the Participant sufficient time to carefully read this Waiver, (2) the Participant has been given the opportunity and has been encouraged to seek independent legal advice prior to signing this Waiver, (3) the Participant fully understands the risks and claims that the Participant is waiving to participate in the Activity, (4) the Participant is freely and voluntarily executing this Waiver, and (5) the Participant is forever prevented from suing or otherwise claiming against the Activity Provider for any property loss or personal injury that the Participant may sustain while participating in or preparing for the Activity.

GOVERNING LAW

7. This Waiver will be governed by and construed in accordance with the laws of the State of New Jersey.

RELEASE OF LIABILITY

8. Although the safety of all sport activities is the primary concern, indoor sport activities at Sport Center's facilities may cause injuries and/or death. I expressly assume the risk of injury, death, and/or illness arising from any cause, and agree to waive the right to pursue any claim against the Sport Center and the persons in charge.
IN CONSIDERATION of being allowed to participate in the Activity and other good and valuable consideration, the receipt of which is hereby acknowledged, I (Guardian/Parent Full Name) blanks* of (Participant Full Name) blank* , agree with James Ofei Ankra-Badu of Extra Mile Basketball NJ LLC (the Activity Provider) to the above Activity Liability Waiver.
PHOTO/VIDEO RELEASE FORM
I hereby grant Extra Mile Basketball NJ LLC (Activity Provider) the irrevocable right and permission to use photographs and/or videos taken of My child (Participant/Player) during the Tryout training session, for promotional, educational, and/or informational purposes, including but not limited to publications, websites, social media, and other marketing materials. I understand and agree that these materials will become the property of Extra Mile Basketball NJ LLC and will not be returned. I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown.I hereby agree to release, defend, and hold harmless Extra Mile Basketball NJ LLC from and against any claims, damages, or liability arising from or related to the use of the photographs and/or videos.
