• Acupuncture Society Course Accreditation Form

    Please fill in all form fields below and attach the required stated course documents and you will be asked to set up an annual auto-renew subscription of £100 per course on PayPal (this is your course accreditation fee, all teachers must become members are charged at £100 each for thier Society membership and should apply individually on our website, your Graduates can also join as members in the same way). You will need a PayPal account to be able to set this up.
  • First we ask you for the details of your primary (main course), and then there is a input box for any supplimentary (additional) courses you wish to be included in the accreditation, all the course manuals and other documents for each course can be uploaded in the uploader with suitable titles to enable us to deferentiate them
