By signing below, I confirm that all information I entered in this form is accurate, true, and based on my own requests. I understand that my consultation fee goes towards my purchase but if I decide not to make a purchase, my consultation fee is non-refundable.

I understand I will be provided progress shots and / or a final look at my set(s) before packaging and shipping if desired. I also understand I can opt to have my set(s) created and shipped without updates so I will be surprised once recieved.

Once creation of my set(s) is started, I cannot request to change the design completely. I must do this during the consultation phase. I will be allotted three (3) digital mock revisions per consult.

I understand I will be alerted of the start of the creation of my set(s) and acknowledge that I will need to be responsive to any communications within a timely manner during this time should any questions or concerns about my set(s) arise.
