WRITTEN STATEMENT
I give permission for my child to attend the TIMEOUT after-school program. I understand that my child will be transported to the program on a school bus or program transportation van and that I will be responsible for picking my child up from the program every day before 6pm or I will be charged a late fee. I give my permission for staff from the TIMEOUT program to discuss my child's academic program with the Metropolitan Nashville Public Schools staff to ensure seamless instruction and tutoring, I also give permission for my child to be photographed and videotaped for use in projects, brochures and media releases.
