  • Although all of the following may not be applicable to you, you must initial them ALL acknowledging your consent to policies and prices listed.

  • * RESERVING YOUR DATE: A nonrefundable retainer of $100 along with completion of this Bridal Service Agreement is required for me to reserve your date on the books. If you wish to pay the retainer with a credit card, click here. In the unlikely event that I am unable to perform the services agreed upon due to an emergency, illness, or unexpected occurrence, an effort would be made to find trusted substitute to fulfill the contracted services. Inability to perform is the only way a full refund of the retainer would be returned to you.

  • *PAYMENT OPTIONS: Acceptable forms of payment for any fees or services can be paid by the following methods:
    -Cash
    -Check (Made out to cash)
    -Venmo (@MeganPatinHebert)
    -Zelle (504-621-8090)
    -Square/Credit Card (A processing fee is applied with this method of payment)

  • I have paid the nonrefundable retainer through  * on Pick a Date*.

  • * ASSISTANTS: If you have a large wedding party and/or you have tight time restrictions you may want to consider this option. My goal will always be to have enough time to provide quality services that ensure your satisfaction. Should an assistant be requested or needed, I will bring a trusted stylist to assist for $200. (Assistants are only applicable for hair services.)

  • * TRAVEL FEE: Your comfort level is a priority on your wedding day. Should an off-site location be better fit than the salon setting, I will accommodate by traveling to a destination of your choosing. Any location other than SOHO Salon will require a travel fee and you would be responsible for providing a suitable chair, table, mirror, and access to electrical outlets (extension cord if necessary). Travel Fee Under 30 Mile Radius from Salon is $150. Travel Fee Over 30 Mile Radius from Salon will be an additional mileage rate along with hotel room, airfare, rental car, parking garage should they be necessary. Travel fee must be paid no later than 60 days prior
    to your event date!

  • *EARLY CALL: For larger parties or those on tight time restrictions, a $50 early call fee is for any start times prior to 8:00 AM.

  • *FINALIZED HEADCOUNT: Final headcount must be provided 14 days prior to the event. If no headcount is provided, I will use the number listed above in this contract and you will be responsible for the cost of all services previously agreed upon. Additions to the headcount will only be possible if time permits but cannot be guaranteed.

  • *LIABILITY: Anyone receiving services releases liability for any claims, damages, costs, or losses—including but not limited to damages, allergic reactions, skin or scalp irritation, burns, rashes, or breakout arising from or connected to the services provided. Any known allergies or sensitivities should be noted before services begin.

  • * HAIR SERVICES: Hairstyle pricing is $65. The only time this price would fluctuate is if there is extra time needed for installing extensions, extremely thick or long hair, or very intricate style designs. To ensure service times remain on schedule, anyone receiving hair services should have clean and dry hair. Once a hair service is completed, any additional styling (e.g., taking a hairstyle down and requesting a new one) will be considered a new service and will incur an additional fee. All restyling services are subject to time and availability.

  • * MAKEUP SERVICES: Makeup applications start at $85. You can upgrade your makeup service with false lashes but this is completely optional. Makeup with false lashes is $95. To ensure services times remain on schedule, please inform anyone receiving makeup services, they should have a clean face with no makeup at the start time of their service. Several brides have inquired so I like to be transparent that I do not specialize in tattoo coverage.

  • Signing below acknowledges that you have read, understand, and agree to all the terms and conditions listed herein.

  • Clear
  • ELECTRONIC SIGNATURES.  The relationship between me and potential client signing above, including  the validity, construction, and enforceability of this Contract, shall be governed in all respects by the laws of Louisiana. This Contract  may be executed in multiple counterparts, each of which shall be deemed an original but all of which together shall constitute one and  the same instrument, and all signatures need not appear on any one counterpart. A signed copy of this Contract delivered by email in  portable document format (.pdf), or by any other electronic means intended to preserve the original appearance of a document, shall  be deemed to have the same effect as physical delivery of a signed original and may be used in lieu of the signed original for all purpos es. The parties understand and agree that the use of an electronic signature or digital signature that is provided and established by the  respective electronic address of the party is acceptable and will be treated as an original of said party’s signature. 

  • Should be Empty:
