We are very grateful for your interest in University Baptist Church’s Mother’s Day Out program! Upon receipt of this form, you are invited to apply for an open spot in a September classroom corresponding to your child’s age.
Positions for new students will be filled on a first come/first-served basis beginning Tuesday, January 30th at 9:00 AM. A Non Refundable Registration Fee of $250 ($225 for second child) must accompany this application. However, your check will not be cashed until we are certain that a space is available. You can mail the registration check (check addressed to University Baptist Church, envelope addressed to University Baptist MDO and including the child's name) or drop it off during MDO hours. Application is complete when both the form and check have been received.
You will be informed by email by Thursday, February 8th, as to whether your child’s enrollment is guaranteed (at which point your check will be cashed) or whether you are on a waiting list (at which point your check will be held uncashed until enrollment). We truly wish that we could enroll each child, but we are dedicated to maintaining small student/teacher ratios in order to provide the best possible care and learning for the children. A $75 per child Curriculum Fee will be due by July 15, 2024 for 2’s,3’s, and 4’s classes. A $75 per child Resource fee will be due by the first week of school for all classes.
